For decades it was assumed inflammation ended passively, by exhaustion. Today resolution is described as a <strong>biosynthetically active phase<\/strong> with its own molecules &mdash; the <strong>SPMs<\/strong> (specialized pro-resolving mediators). Some of them, the lipoxins, come from arachidonic acid; the others &mdash; <strong>resolvins, protectins and maresins<\/strong> &mdash; your body builds from the <strong>EPA<\/strong> and <strong>DHA<\/strong> in your omega-3. And one detail changes everything: <strong>that program starts within the first hours<\/strong>, not at the end. With no raw material on hand, the program has nothing to run on.<\/p>\n<\/div>\n<p>You cut your hand. A few days later the area doesn&#8217;t hurt, the redness is gone, and skin is skin again. It was a reasonable idea. It was also wrong.<\/p>\n<p><strong>Charles Serhan<\/strong>&#8216;s group at Harvard described resolution as <strong>an active, coordinated program<\/strong> that <em>starts within the first hours<\/em> of the inflammatory response<sup><a href=\"#references\">1<\/a><\/sup>. It isn&#8217;t what&#8217;s left over once inflammation ends: it&#8217;s a process of its own, with its own phases, molecules and timing. Resolution is now described as a <strong>biosynthetically active phase<\/strong><sup><a href=\"#references\">3<\/a><\/sup><sup><a href=\"#references\">5<\/a><\/sup>.<\/p>\n<div class=\"vf-note\">\n<p style=\"margin:0;\">It&#8217;s the difference between <strong>putting a fire out<\/strong> and <strong>closing the site down<\/strong>. Firefighters put the fire out. But if nobody clears the burnt material and repairs the damage, the area stays black. Your body has one crew that puts it out <em>and<\/em> another that clears and rebuilds &mdash; and they are different processes.<\/p>\n<\/div>\n<h2><span class=\"ez-toc-section\" id=\"The_resolution_program_in_five_steps\"><\/span>The resolution program, in five steps<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>This is what happens, in order, when inflammation truly resolves<sup><a href=\"#references\">1<\/a><\/sup>:<\/p>\n<div class=\"vf-step\">\n<p style=\"margin:0;\"><span class=\"vf-fase\">1<\/span><strong>Stop calling in reinforcements.<\/strong> Neutrophil recruitment halts &mdash; those are the cells that arrive first at the site. While they keep coming, the process can&#8217;t close.<\/p>\n<\/div>\n<div class=\"vf-step\">\n<p style=\"margin:0;\"><span class=\"vf-fase\">2<\/span><strong>The ones that arrived stand down.<\/strong> Their programmed death &mdash; apoptosis &mdash; is triggered, in an orderly way and without damaging the surrounding tissue.<\/p>\n<\/div>\n<div class=\"vf-step\">\n<p style=\"margin:0;\"><span class=\"vf-fase\">3<\/span><strong>Someone clears the debris: efferocytosis.<\/strong> Macrophages engulf those spent cells. Engulfing is <strong>phagocytosis<\/strong>, the general mechanism by which a cell eats something &mdash; a microbe, a particle. But when what it eats is the body&#8217;s own apoptotic cells, it has its own name: <strong>efferocytosis<\/strong>, from the Latin <em>effero<\/em>, &#8220;to carry out for burial.&#8221; That&#8217;s not a vocabulary detail: without that clearance, the debris keeps the alarm ringing and the process cannot close.<\/p>\n<\/div>\n<div class=\"vf-step\">\n<p style=\"margin:0;\"><span class=\"vf-fase\">4<\/span><strong>The order to repair goes out.<\/strong> That same clearance releases the signals that set the tissue rebuilding. Clearing and repairing aren&#8217;t two things in a row: one triggers the other.<\/p>\n<\/div>\n<div class=\"vf-step\">\n<p style=\"margin:0;\"><span class=\"vf-fase\">5<\/span><strong>The process is declared over.<\/strong> Macrophages leave the area and the tissue is back to what it was. The inflammation hasn&#8217;t been switched off: it has been <em>resolved<\/em>.<\/p>\n<\/div>\n<div class=\"vf-note\">\n<p style=\"margin:0 0 8px 0;\"><strong>Three names worth keeping straight<\/strong><\/p>\n<ul style=\"margin:0;\">\n<li><strong>Apoptosis<\/strong> &mdash; <em>programmed<\/em> cell death. The cell shuts down in an orderly way, without rupturing or spilling its contents. That&#8217;s step 2.<\/li>\n<li><strong>Phagocytosis<\/strong> &mdash; the <em>general<\/em> mechanism by which a cell engulfs and digests something: a microbe, a particle, a fragment.<\/li>\n<li><strong>Efferocytosis<\/strong> &mdash; the <em>specific<\/em> phagocytosis of the body&#8217;s own apoptotic cells. That&#8217;s step 3, and it&#8217;s the one that hauls away the rubble so the tissue can be repaired.<\/li>\n<\/ul>\n<p style=\"margin:8px 0 0 0;\">Put simply: <strong>efferocytosis is a kind of phagocytosis<\/strong> &mdash; the one that deals with your own cells &mdash; and it only makes sense after apoptosis. The three run as a chain, not in parallel.<\/p>\n<\/div>\n<p>None of these five steps is automatic. Every one of them needs someone to give the order.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"The_beginning_programs_the_end\"><\/span>The beginning programs the end<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Here&#8217;s the detail almost nobody talks about, and it&#8217;s so good that the scientific paper itself is titled after it: <em>&#8220;Resolution of inflammation: the beginning programs the end&#8221;<\/em><sup><a href=\"#references\">1<\/a><\/sup>.<\/p>\n<p><strong>The shutdown program doesn&#8217;t start when inflammation ends. It starts at the same time inflammation switches on<\/strong>, within the first hours. While the site is still in full swing, how and when it will close is already being decided.<\/p>\n<p>And there&#8217;s a changing of the guard: the mediators that dominate early on &mdash; prostaglandins and leukotrienes &mdash; give way to other molecules, the <strong>lipoxins<\/strong>, which begin the termination sequence<sup><a href=\"#references\">1<\/a><\/sup>. The same response that switches the process on is the one that, if all goes well, sets up its ending.<\/p>\n<div class=\"vf-note\">\n<p style=\"margin:0;\">The practical consequence is uncomfortable but clear: <strong>your body closes the process with the material it already had<\/strong>, not with what you give it once something already hurts.<\/p>\n<\/div>\n<h2><span class=\"ez-toc-section\" id=\"What_the_program_is_made_of_the_SPMs\"><\/span>What the program is made of: the SPMs<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The molecules running all of this are called <strong>SPMs<\/strong>: <em>specialized pro-resolving mediators<\/em>. They&#8217;re a family that includes <strong>lipoxins<\/strong>, <strong>resolvins<\/strong>, <strong>protectins<\/strong> and <strong>maresins<\/strong><sup><a href=\"#references\">2<\/a><\/sup><sup><a href=\"#references\">3<\/a><\/sup>.<\/p>\n<p>They work as <strong>stop signals<\/strong>: they terminate the inflammatory response once it&#8217;s no longer needed and actively promote tissue repair and regeneration. As <strong>Fredman and Serhan<\/strong> describe it, their reported actions also include <em>limiting pain<\/em><sup><a href=\"#references\">4<\/a><\/sup>. They don&#8217;t block anything: they <em>order the close<\/em>.<\/p>\n<p>And where do they come from? <strong>Resolvins, protectins and maresins are built from your omega-3<\/strong> &mdash; the EPA and DHA<sup><a href=\"#references\">2<\/a><\/sup><sup><a href=\"#references\">3<\/a><\/sup> &mdash; and they shorten the time neutrophils spend at the site<sup><a href=\"#references\">1<\/a><\/sup>.<\/p>\n<div class=\"vf-note\">\n<p style=\"margin:0;\"><strong>This is where omega-3 fits into the whole story.<\/strong> It isn&#8217;t an anti-inflammatory that shuts things down from the outside: it&#8217;s the <strong>raw material<\/strong> your body uses to build the molecules that close the process. With no substrate available, the program has nothing to run on. We lay out the full chain on <a href=\"\/us\/science\/\">the science page<\/a>.<\/p>\n<\/div>\n<h2><span class=\"ez-toc-section\" id=\"What_happens_when_inflammation_doesnt_resolve\"><\/span>What happens when inflammation doesn&#8217;t resolve<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>When the resolution response isn&#8217;t effective or doesn&#8217;t arrive in time, inflammation can become <strong>chronic<\/strong>: instead of closing, it stays in the background, at a low level, for months or years<sup><a href=\"#references\">4<\/a><\/sup><sup><a href=\"#references\">3<\/a><\/sup>. It&#8217;s what we usually call <strong>chronic inflammation<\/strong> &mdash; and seen this way, it isn&#8217;t inflammation that turned permanent, but inflammation that <strong>never got resolved<\/strong>.<\/p>\n<p>This has changed how researchers look at persistent inflammatory situations. One well-documented example, and we quote it as published science rather than as anything to do with a supplement: atherosclerosis is described today not only as inflammation of the arterial wall, but as <em>an unresolved inflammatory condition, lacking the switch from the pro-inflammatory to the pro-resolving phase<\/em><sup><a href=\"#references\">6<\/a><\/sup>.<\/p>\n<p>Put another way: <strong>in many cases the problem isn&#8217;t that it switches on.
